DOCX templates the coach fills before, during, and after every 1:1. Same structure across all coaches so the platform can roll up anonymised outcomes for the Q1 board report.

1. Intake form (first session)
Used in session 1 only. 60-minute session. Coach completes form WITH the learner, not after. The act of filling it together IS the work of session 1.

Storage + retention
- All four templates submit to the platform under the hashed learner ID.
- Learner's name is NEVER joined to the case content unless the learner has explicitly consented for follow-up.
- Retained for 7 years from last session (insurance + safeguarding minimum). Deleted thereafter.
- Learners may request access or deletion under standard data-rights process.
